Payment is due at the time of service. We accept all major credit cards including FSA/HSA
Membership does not cover meal replacements
Medication will be prescribed routinely and coverage will depend on your health insurance benefits
You may submit paperwork to your health insurance for possible reimbursement of medical services. Your doctor can provide during your visits
It is the member’s responsibility to obtain approval from their benefits administrator of terms of membership fee reimbursement and coverage.
